The Questions to Ask Before Hiring a Home Care Agency in Missouri
Katie Stippec, Co-Owner and Operator, Here at Home Healthcare · August 11, 2026
You can learn more about a home care agency from six direct questions than from an hour of brochures. Ask who employs the caregivers, what happens when one calls in sick, who answers the phone at 2am, how caregivers are screened, what the rate includes, and how the agency limits its caseload. The answers separate agencies quickly.
We are one of the agencies you might interview, so read this knowing where it comes from. These are the questions we would ask for our own parents, and we have included what a good answer sounds like.
1. Are your caregivers employees, and are they bonded and insured?
Some operators are placement services that match you with independent contractors. The price looks lower until you realize you are the employer, with the payroll, liability, and supervision that entails. Ask directly: W-2 employees or contractors? Bonded? Covered by the agency's insurance if something breaks or someone is hurt? At Here at Home, every caregiver is a bonded, insured, background-checked member of our team.
2. What happens when a caregiver calls in sick?
This is the question that predicts your Tuesday mornings. A good agency staffs the replacement and tells you it is handled. A weak one leaves you to scramble. Ask how often shifts go unfilled and what the backup process actually is, not the ideal version.
3. Who answers your phone at 2am?
Call the after-hours line before you sign anything, and see what happens. An answering service that takes a message is not the same as reaching a person who can act. Our line, 314-708-8716, is answered live by our team around the clock, no 800 number, no phone tree. We built the company that way because care questions do not keep business hours.
4. How do you screen and match caregivers?
Minimum: criminal background checks on every hire, no exceptions. Then ask how they decide who walks into your mother's house. Matching on personality and routine matters as much as availability; a caregiver your parent likes is a caregiver your parent lets help. Ask what happens if the match is not right, and whether the same people generally come, or strangers rotate through.
5. What does the hourly rate include, and what costs extra?
Get it in writing: wages, payroll taxes, insurance, supervision, scheduling, backup coverage. Ask about shift minimums, weekend and holiday premiums, and mileage. The cheapest quoted rate is not always the cheapest month. We covered current St. Louis market rates in our cost guide if you want the lay of the land first.
6. How many clients do you take?
Growth-mode agencies say yes to everyone and quality follows census downward. Ask if there is a cap. We hold our weekly schedule to about 500 hours of care on purpose; fewer clients means our team stays focused and caregivers arrive prepared. Whoever you hire, listen for whether anyone has thought about the question at all.
One thing you do not need to ask about in Missouri
Missouri does not license private-pay, non-medical home care agencies, so "are you licensed?" has no meaningful answer here for this kind of care. The trust markers that do exist are the ones above: employment model, bonding and insurance, screening, and how the operation behaves when you test it.
Common questions
How many agencies should we interview? Two or three. Past that, the comparison blurs. Take notes against the six questions and trust the differences you observe, like who called back first.
Should my parent meet the agency before we decide? Yes. Watch how the assessor treats your parent, whether they talk to them or over them. That is the culture you are hiring.
What if we start and it is not working? Ask about contract terms before signing. You should be able to adjust the schedule or stop without a penalty gauntlet.
